New Patient Registration

New registrations are welcome here at Calder View Surgery, we do have a practice area boundary to adhere to so please check with our reception staff who will be happy to advise whether your address is within our catchment area.

To register you will need to collect a registration form per person which will need to be returned with the following items:

  • Photo ID (passport, driving license)
  • Proof of address (utility bill, bank statement)
  • Repeat medication slip

A birth certificate will be accepted if photo identification is not able to be provided.

Your NHS number will be required to ensure your registration is completed correctly, this can be found on a repeat prescription or by ringing your old GP practice, if you are new to the country and this is your first registration within the UK you will not have one of these.

Please note registrations are not always completed on the same day forms are handed in at reception and may take a few days to complete.
