For prescription requests please call us to sign up for Systm 1 online or you can email a prescription request to calderview.patient@nhs.net stating your name, date of birth and exact details of medications you require. If you do not have access to email or the internet if you can at all please ask a family member or friend to do this on your behalf.

Due to events and the risk of spread of corona virus please read the information below carefully
If you start with a new cough or fever or lost your sense of taste or smell
DO NOT ATTEND THE SURGERY OR A&E
If you have mild symptoms you need to self isolate you do not need to inform medical services
If you feel that you cannot cope with your symptoms or are unwell for more than 1 week, or you feel you are getting worse,contact NHS 111 online or by phone if you have no access to the internet
If you turn up at the surgery with these symptoms we have been told to immediately finish the consultation and to send you home to contact 111. Doing this could result in the surgery room been out of action for that day .

No patient will be coming to surgery without been triaged by a GP .
If you ring for an appointment our admin team will take details of why you wish to see a gp you will then at some point be rung back by one of the GP's and if necessary they will make you an appointment to be seen.
Please be honest about your symptoms if you are not you are potentially risking the health of all our staff .

FLU IS BEEN DELIVERED DIFFERENTLY THIS YEAR THE MAJORITY OF PATIENTS WILL BE ASKED TO ATTEND OUR DRIVE/ WALK THRU FACILITY . APPOINTMENTS ARE AVAILABLE TO BOOK BY RINGING THE SURGERY ON 01924 767266 .

THERE ARE SEPERATE DATES FOR OVER 65'S AND UNDER 65'S AT RISK
CHILDREN AGED 2 & 3 WILL RECEIVE A NASAL SPRAY AND WILL BE BOOKED TO COME INTO SURGERY
THE GOVERNMENT HAVE EXTENDED THE COHORT OF PATIENTS THAT SHOULD RECIEVE A FLU TO ALL OVER 50'S. UNFORTUNATELY THERE ARE CURRENTLY NO STOCKS AVAILABLE TO VACCINATE OVER 50'S IF AND WHEN THERE ARE FLU'S LEFT AFTER ALL AT RISK GROUPS ARE VACCINATED WE WILL CONTACT YOU TO OFFER YOU A VACCINATION .

Routine Evening and Weekend Appointments
Evening and weekend appointments now available run by Curo Health our local GP federation
Patients can now see a local GP for routine appointments on weekday evenings and at weekends.
Pre-bookable and on-the-day appointments are available between 6.30pm and 9.30pm each weekday, with additional slots available on Saturdays from 9am and 4pm and Sundays 9am and 1pm.
The appointments will be available in Dewsbury Health Centre.
How to book an evening or weekend appointment
There are ‘pre-bookable’ and ‘on-the-day’ appointments which can be accessed through your own GP practice, or by calling the free NHS 111 phone line out of normal working hours
The practice offers extended hours appointments these are appointments which are outside the time our normal surgery .

To check in for appointments outside these hours use the Check in screen located in reception.
If you are late for your appointment the check in screen does not allow you to check in please knock on the door. But be aware staff are busy dealing with telephone calls so may take some time to attend to you

We are also advising all our patients that National guidance asks practices to give everyone an accountable GP, this simply means that all patients will have a named GP in their record it does not in anyway mean that you cannot choose to see the GP of your choice when you come to surgery.
We are always looking for new members at our patient participation group, this group meets quarterly and has input into the running of the practice, also supporting the practice when we look to provide new services. Please ask at reception if you would like to get involved .

Out of hours services are provided by North Kirklees Comissioning Group (NKCCG)from 6.30pm to 8am weekdays and from 6.30pm Fridays to 8am on Mondays.Should you require a Dr urgently within these tims please call 111.
